When you have a small team, and you're trying to grow into a big team, you can become susceptible to team indigestion - the new people overwhelm the incumbents, and you lose all the previous structure.
Which is like making a risotto. If you pour a litre of stock into the rice in one go, it won't work. You need to make sure that each new addition is absorbed before you add the next one.
